Our Optional 60-Day Aftercare Program for Addiction
On the road to a sustained and lasting recovery, a solid aftercare program is oftentimes helpful in maintaining a healthy, sober lifestyle.
At Evolutions, we offer support for our Clients
after Primary Treatment or even after a period of Extended Care.
Our 60-day Aftercare Program may consist of the following:
Weekly one-on-one outpatient meetings with an Evolutions Counselor who will check in on the Client’s progress at reestablishing his or her life in the community
Weekly support groups where the Client will meet with an Evolutions Counselor and peers in the recovery community to share their experiences, challenges and successes as they continue on their path toward recovery
Quarterly check-ins where we monitor Client progress and provide support and guidance in any areas where Clients may be struggling
IOP and Transitional Housing
We also offer day or evening IOP with housing and a 2 month transitional program that includes housing, support, supervision, ongoing clinical groups, etc. This is far more than sober housing and our housing is Joint Commission Accredited as well as our clinical services. Clients can go back to work, school, or volunteer up to 20 hours a week while re-integrating into the community and life in this 2 month aftercare program.
At Evolutions we believe that continued care and observation made possible by a structured Aftercare Program provides every Client with the extra support, encouragement and guidance they will need to lead an independent and lasting sober life.
What is Outpatient / Aftercare?
This program is a supplementary but vital component of the Evolutions treatment process, as it increases the likelihood of sustained success in recovery by facilitating a healthy continuation of treatment and support in a less intensive format.
This level of care is created specifically for patients who have completed the entire continuum of care from Partial Hospitalization to Intensive Outpatient. Previous treatment levels can be very different from the reality of daily life and its temptations and quite daunting for many without continued support.
Moreover, in this final stage of treatment, patients are supported in their new sober life by continuing to participate in a minimal amount of weekly treatment services that support them and help them cope with the situations they face on a daily basis as they transition back into a healthy and productive lifestyle.
