**2. Service Plans:**

  1. Individuals with disabilities who require ongoing accommodations and support will have individualized Service Plans developed to meet their specific needs.

  1. Service Plans will be collaboratively developed with the individual, their legal guardian (if applicable), and treatment team members. These plans will outline goals and strategies tailored to address the individual's unique needs.

  1. The Service Plan will be reviewed and updated regularly to ensure that it remains relevant and effective in meeting the individual's evolving needs.

  1. Instances of non-compliance will be promptly addressed, and corrective actions will be taken as needed.

**5. Accessibility Coordinator:**

  1. Evolutions Treatment Center will designate an Accessibility Coordinator responsible for overseeing and implementing this policy, ensuring compliance with federal and state requirements.

  1. The Accessibility Coordinator will serve as the primary point of contact for individuals with disabilities and their requests for auxiliary aids and Service Plans.
